Co-produced support plans
We develop every support plan with you — not for you. Your goals, your routines and your vision for independence shape every decision we make.
DeRivian Care provides supported living services for adults across London — helping people with learning disabilities, autism, mental health needs and physical disabilities to live independently, build life skills and be part of their community.

Why choose us
We listen first, then act. Every person we support has their own story, their own goals and their own vision for how they want to live — and we build our support around that vision, not the other way round.

We develop every support plan with you — not for you. Your goals, your routines and your vision for independence shape every decision we make.

A small, consistent team builds the real relationships that make progress possible. We invest in our staff so they stay — and so you always have people who truly know you.

We step back where we can, encourage where we should, and support where it counts — so you grow in confidence and live life on your own terms.

We respect your culture, your language and your faith. London's diversity shapes how we work — every support plan honours the whole person, not just their assessed needs.

In supported living, you hold your own tenancy and live in your own home — a self-contained flat, a shared house, or your existing property. We provide the support you need within that home, but your accommodation and your support are separate. You have full tenancy rights and are never 'placed' anywhere.
We co-produce every support plan with you — not for you. We sit down with you, and anyone you choose to involve, to understand your goals, your routines and what independence looks like to you. The plan is reviewed regularly and updated whenever your needs or ambitions change.
We provide supported living services across London and surrounding areas for adults aged 18 and over. If you're unsure whether your location is covered, get in touch and our team will let you know straight away.
We understand that transitions can be time-sensitive. We work with you, your social worker and any commissioning body to put support in place as promptly as possible — always without cutting corners on getting to know you properly first.
Yes. We work closely with local authority commissioners, social workers, NHS continuing healthcare teams and direct-payment holders. Our team is experienced in referral and funding pathways and can guide you through the options available.
